We see all patients on an appointment basis and ask that you send a message using the form below so that we may reserve time for you. In order to reserve your First Office Visit appointment time for any of our practitioners, your credit card number will be required as a deposit.
New Patient Forms
It is important that we receive your intake paperwork at least 24 hours prior to your initial visit in order to maximize our time together. It is also helpful to bring, fax, or email any recent laboratory results that you have had done by other doctors. If you have online access to your previous lab results please bring your user ID and password so the results can be reviewed during the visit. If you do not have copies of your lab results we can request them after your initial appointment is complete
The link below will take you to an online fillable form that will be automatically emailed to us upon completion. This method allows us to review your case prior to your initial visit. Please bring any pertinent lab work to the initial appointment.

Cypress Natural Medicine is located at 359 Middlefield Road in downtown Palo Alto, California. The clinic parking lot entrance is on Lytton Avenue. If the parking lot is full there is plenty of free street parking.

Appointments cancelled with greater than 48 hours notice will incur no charge. The full office visit fee will be charged for appointments cancelled with less than 48 hours notice.
Telephone consultations are available for First Office Visits and Return Office Visits. Standard office visit fees apply to all telephone consultations.
We make every effort to minimize the cost of your medical care. Full payment is required at the time of service. Payment plans are available for patients with financial hardship. We accept payment by cash, check or credit card (Visa or Mastercard). Checks or charges that are denied for lack of funds will incur a fee of $35.00 per transaction. Each year there will be slight increases in clinic fees due to increases in expenses. We are committed to providing economical, quality health care, and appreciate your patronage.
Our clinic does not directly bill insurance for medical services provided. Upon request, we will provide patients with the appropriate documentation (Superbill) required to submit a claim to their insurance company. We have found that many PPO plans provide partial to full reimbursement for services provided. Our patients often utilize their flex spending account associated with their primary insurance in order to pay for office visits. Other patients have utilized an HSA (health savings account) that contains pre-tax funds to be used toward medical care that is not standardly covered by their primary insurance plan.
